If you would like to pay for a museum membership or make a donation, you can submit your payment by credit card using our Credit Card Authorization Form.

How to Submit Your Payment

Fill out all sections completely.
Sign and date the form.
Mail the completed form to:

For security purposes, we cannot accept credit card authorization forms by email. All forms must be mailed to the museum for processing.
